Accountable / Named GP for All Patients

All patients registered at Forton Medical Centre have a named doctor who has overall responsibility for your care and support. If you choose to see another doctor at the surgery you are entirely free to go on doing so exactly as before.

How to register

If you wish to register with us you can complete the Registration Pack available to download below or call in at the surgery to complete the forms. Please note you will need 2 forms of ID, a driving licence or passport and also a utility bill or similar to confirm your address.  We can then offer an appointment for a simple registration medical with one of our nursing team. Please bring a urine specimen and a list of your medication with you when you attend this appointment.


Pre-Registration

Please complete the below 'Registration Form(s)' and hand them in to the reception team.  

You are required to bring in 2 proof of address documents along with you.

Children 16 and under do not need to complete the registration form. An adult member of the family can register them by providing the required information and signing the registration application on their behalf.

As a patient, you have the right to express preference as to which GP you wish to see. However, if you require an appointment and your preferred GP is not available, you will be offered an appointment with another doctor. If you still wish to see your preferred GP you may have to wait longer.

You can also visit the surgery and the above forms will be provided.

There is easy wheelchair access to all consulting rooms and toilets.
